New Patient Registration Form
To register as a patient with Cairn Medical Practice please complete a registration form and email it to us at nhsh.gp56006-admin@nhs.scot. You can also drop the form off at our reception.

Practice Boundary Area

Patients wishing to register with our practice must reside within our Practice Boundary Area. We cannot register patients who live out with the demarcated boundary. In the main, this is because we cannot provide our contracted service out of hours or provide home visits to you if you live far away.
Our Practice Boundary Area was reduced effective 1st January 2026. This change will not affect existing patients where your registered address is reflected in your medical record on or before 31st December 2025 and now falls out with our updated Practice Boundary Area.
However, if you are an existing patient and you move to a new home outside of our updated Practice Boundary Area after 31st December 2025 then you will need to register with a different GP Practice closer to your new home. We will allow a period of 2 weeks for you to do this.
